Fatty deposits on EPSM Shire

I have a 6 yo Shire gelding who biopsy came back neg for EPSM(Dr. Valentine read), but exhibits all the signs of EPSM.
He has fatty deposits above his shoulder unlike I've ever seen on another horse. Is this because of all the veg oil he is getting? It is like a smushy cantelope, symetrical above the shoulders and with a saddle on, looks like it pushes forward.

His condition has worsened in a couple of ways, from what I read, are atypical...a lot of up front issues now accompany the hindend.
I feel as though I am doing everything I can. He doesn't typically take a lame step. He cannot back off a trailer, standing asleep, he will suddenly straighten one front leg, then the other...I've never seen anything like this and neither has my vet, nor farrier, nor draft friends...
ANY HELP would be extremely appreciated

Having fatty deposits like this is definitely not healthy. I worry that he may be responding to a high fat diet by developing insulin resistance rather than becoming healthier. Given that he is not improving I'd recommend cutting oil intake at least in half. Talk to your veterinarian about adding thyroid supplementation for a period of time to try to "jump start" his metabolism to get rid of fat pads like this. Continue to feed a low starch and sugar diet and try to exercise him as much as possible. There is a new product being tested for insulin resistant horses and also for EPSM horses. It is l-acetyl carnitine, sold for people. Doesn't hurt and might help. Recommended doses I have seen for horses are from 1000-2000 mg per day. I'd suggest going with the 2000 mg per day for a bit and see if that makes a positive difference. Good luck and do let us know how things go.
