Extreme hoof length

I have this client with a miniature-shetland cross. She calls me when the hooves are real long. This time she calls me after almost a year. The heels are always crushed, seedy toe, probably white line problems. What would be the best course for me to take? How long should the hooves be?

There is not an exact length that I can state for a pony that would cover all of them, but most will be somewhere around 2.5 inches when trimmed. When I have a customer who won't schedule, I charge them extra. This means that if they should have had the horse trimmed 3 times over the course of 6 months, I will charge 3 times the going rate. It costs them the same whether they schedule or not.
